What measures can be taken to avoid capsular contracture?

Capsular contracture is one of the possible long term complications that could arise from a breast augmentation procedure. However, its appearance following such a procedure is not a phenomenon that is fully understood.

It is a well accepted fact that placing breast implants under the pectoralis (chest) muscle significantly reduces the incidence of capsular contracture, compared to placing breast implants on top of the chest muscle. Moreover, it is known that saline breast implants typically tend to have a lower incidence of capsular contracture in comparison to silicone implants. However, this difference is not as significant as it used to be with the old silicone implants, which had a very high incidence of hard capsule formation.

Interestingly, patients should know that there isn’t any good evidence to support the idea that the use of breast massage following a breast augmentation procedure lowers the risk of capsular contracture. Furthermore, taking a high dose of vitamin E hasn’t appeared to successfully treat or prevent the condition either.

Another long term risk following a breast augmentation procedure is loss of sensation of the nipples. However, patients should not confuse this long term numbness complication with the temporary numbness or the hyper-sensitivity that they may experience in their nipples immediately after the procedure which can last for weeks or months.

The more serious complication is when a patient experiences long term loss of sensation in the nipples. This happens in 10 to 15% of breast augmentation procedures. The loss of feeling in the nipples may be linked to the size of the breast implant used in the procedure. When a large implant is placed in a patient, a similarly large pocket needs to be created. As a result, the surgeon has to perform a more extensive surgery, which could increase the risk of numbness to the nipples.
